Instagram users reported a surge in violent and NSFW content despite having Sensitive Content Control enabled. Meta acknowledged the issue, calling it a “mistake” that led to excessive recommendations on Reels.

A spokesperson apologized, confirming that a fix is in progress. Meta’s policies aim to filter disturbing content, allowing some graphic posts only when highlighting social issues, with warning labels applied as needed.
